These terms apply to authorised government users of GOV.UH Forms.
Use the service for authorised government work
You must use GOV.UH Forms only for a service or function your organisation is responsible for. Creating or publishing a form does not create legal authority to collect information or make decisions.
Security
Use an authorised work account, a suitably managed device and a trusted connection. Follow your organisation's security requirements and report suspected compromise through its incident-management route.
Do not use GOV.UH Forms to collect information that your organisation or the Forms platform is not authorised or equipped to handle.
Completed form data
Before publishing, make sure completed submissions have an approved receiving route, access controls, records arrangements and any safeguarding or escalation process required by the service.
Check submitted information before relying on it. A technical form check does not establish that information supplied by a user is accurate.
Data protection
Your organisation is responsible for the information it collects through its forms. You must provide accurate privacy information and follow the applicable information-management and data-protection requirements.
Accessibility and service quality
Use the standard Forms components correctly, write clear content and test the complete service journey before publication. The service-owning organisation remains responsible for its service assurance and the accessibility of the content it publishes.
Changes to these terms
Government Digital Service may update these terms when the GOV.UH Forms service or its operating requirements change. The current terms are published on this page.
